Get a Membership Form
Pick one up at our shelter during open hours (Saturdays 11am–1pm) or use our contact form to request one.

Complete and Submit
Fill out the form and submit it with your membership fee using one of these methods:

- In person: In person: During shelter hours at 1530 N Lowell Avenue
